The RTP Handoff Card
One page you fill out and hand to the coach, so they know what your injured athlete can do today without stopping a session to ask you. One sheet covers one phase. When the phase goal is met, you write the next one.
Where should I send it?
You'll get the fillable card and a video of me filling one out on a real case.
What's in it
The blank card as a fillable PDF. What you're protecting. The diagnostic tests with where they started and what has to go negative to move on. The targeted limiter and what comes next. What the athlete may do and must not do. What's protecting the injury during training. The response rules for green, yellow and red. This phase's rehab with its doses.
A video of me filling one out on a rhythmic gymnast with a Jones fracture and a first metatarsal avulsion fracture, four weeks before she flew to the national championship. What goes in each box, and why the coach needs it there.
It's an asynchronous tool. You write it, they read it, nobody has to find each other between sessions.
